My take-away from my 10-day Vipassana retreat is that their style of Vipassana body scan is meant to interrupt the pattern of "negative sensation" -> "reaction." By training yourself to observe "negative sensation" without judgement, you can basically short cut the process of "tensionā at a fairly early stage. Another thing it can do is help you with the general "awareness of body" that may have been damaged by chronic pain. Vipassana and related meditation is trying to work at the more "root" causes of the postural and other issues rather than the more physical adjustments of other therapies. |

In regards to trying to fix sleep apnea and other jaw-related issues, there are some alternatives to jaw surgery or supplements to it. Very few of these except apnodent are true āreplacementsā in that their effects are likely smaller than a full jaw surgery or quasi surgical expanders.
Ā 7. Checking your dwelling for mold issues. Mold allergies can be very disruptive to sleep and cause a number of other problems. It's worth having a plan for checking and destroying mold if it arises in one's house. Ā 8. Maxillary Skeletal Expanders

A Maxillary Skeletal Expander (also known as MSE), is a class of devices that use at least 4 screws also known as Temporary Anchorage Devices / TADs to attach to the upper palate. While itās not considered a āsurgery,ā the screws generally need to be inserted by a surgeon under anesthesia. It works by separating the mid-facial bones that were previously two separate bones, but fused together at the mid-palatal suture. The MSE is a newer version of Ā Miniscrew-Assisted Rapid Palatal ExpansionĀ (MARPE), which is a type of Rapid Palatal Expansion (RPE).
Rapid palatal expansion historically used teeth-based forces and only worked in teens. Trying to do tooth-borne-only appliancesĀ in adults would likely only damage teeth. An MSE generally uses only bone-based forces and can work in adults. There are MSE variants such as MANEĀ which uses both screws and teeth attachments to exert forces. The last one is what I had done and my experience has been fairly positive. |

Jaw Surgeries and orthodontic expanders for sleep apnea purposes are very complex topics with many approaches and branches with slight variations. A lot of procedures are relatively new, used by only a few doctors. Procedures frequently go through refinement over time, sometimes getting new names. There does not seem to be a good central repository of all the possible surgeries and variations, which makes communication somewhat difficult. I am mainly considering the purpose of surgery being improvement in sleep apnea, sleep breathing as well as some potential postural and symmetry benefits. There are frequently aesthetic benefits (or looksmaxxing) to jaw surgeries or post / pre surgery teeth alignment work, however I donāt focus on those. Surgically Assisted Rapid Palatal Expansion (SARPE) is the oldest and most invasive version and is likely not needed anymore. While itās possible to use it with a tooth-borne appliance, the overall invasiveness is much higher and results are likely similar Distraction Osteogenesis Maxillary Expansion (DOME)Ā is the newer version of SARPE which makes fewer cuts. DONE is the version of DOME, which aims for nasal expansion as well. Many of the procedures mentioned aim for nasal expansion, so the differences are likely minor. Maxillomandibular advancement (MMA)Ā is a combination of Lefort 1 Osteotomy for the upper jaw and Bilateral Sagittal Split Osteotomy (BSSO). The upper jaw is cut from the rest of the skull and then held in place using either plates or wires. The lower jaw is cut on both sides and moved forward after which itās help together by screws and/or plates. See imageĀ Both components of MMA are quite invasive. As a whole MMA is one of the most invasive jaw surgeries with a very long recovery time. It is highly worth exhausting all available alternatives, both less invasive surgeries mentioned here, various devices, night-time devices, Neuro-Cranial Restructuring and postural correction, before coming to this path. |

Postural Restoration Institute (PRI) has one of the most interesting perspectives on posture. They looks at "left right asymmetry" as the core postural issue. Left right assymetry frequently takes the forms of pelvic tits, rib cage twists and shoulder twists. Together they produce a potentially large amount of tension.

Left right asymmetry can be originally caused by many things, such as using right hand too much over the left in manual or computer work. This can result in asymmetric breathing and tense breathing that uses shoulders and neck too much instead of the diaphragm. The whole set of ideas it really worth checking out. This channel is a good starting point.
There are many symptoms of whether you need PRI and are suffering from the left-asymmetry they tend to focus on. some examples:
i) you seem to have a chronic clusterfuck of tension around the left shoulder blade (or any un-even back pain for that matter)
ii) one of your shoulders is visibly different from another (higher or tilted forward differently)
iii) you sit at the computer a lot and use the mouse.
iv) after trying the most basic PRI exercise you feel amazing.
v) your neck has a lot of trouble bending or rotating sideways.
vi) you find that during mediation your body "relaxes into" a twist.
In essence, the important thing to watch out for is not "just" forward head posture or slouches, but rather "asymmetry," which can manifest as holding very tense "twists" of your body. For example, I tend to have pelvic twist to the right and a rib-cage twist to the left, which is a common anti-pattern addressed by PRI.
PRIās recommendations of what to do about this are a form of physical therapy, where they recommend specific exercises to correct the situation. You can find the basic oneās here, but many of the other exercises are going to be slightly specialized to your case. Many of PRI's recommendations are fairly intuitive, they frequently ask you to move your body into the opposite position of your chronically stuck position and breathe deeply to improve mind body connection.
PRIās view is that postural issues and accompanying muscle tension are affected by basically every interaction with have with the environment. Jaw tilts or open bite, for example, can wreack havoc throughout the body. Another issue that I had was bad shoes that were too narrow. I also used to rock climb, using very tight shoes. All of those cause toes to bend inwards, which creates tension in one's legs and contributes to pelvic tilts. My solution was to get better shoes and use a toe spreader from time to time.
